MY WORKING PRACTICE – Conditions for a working contract.
The following outlines the basis of our work together and provides useful information about location and parking. Please read carefully and raise any points you would like to discuss.

Please note, 22a is the right hand doorbell.
PARKING
The surrounding roads are in a controlled parking zone from 10am until 7.30pm. If you require parking within these hours, please inform me via text message within 1 hour of the session and I will arrange for your parking via ‘RingGo’ app. I will respond by text to confirm this. Please include your car registration number. The cost for this is £1:50 per hour.
Please note, the parking session will begin 5 minutes prior to the session to allow time to arrive and park. The parking will expire 5 minutes after the end of our session. I cannot Guarantee a parking place.
SESSIONS

We agree to work at a specific time, once or twice weekly, (negotiable for supervisees).

Individual sessions last for 50 minutes.

In order that people can arrive and leave in privacy, please arrive on time.

If you arrive late, I will not be able to offer an extended session.
PAYMENT OF FEES

A fee of £75 can either be made in cash at the beginning of the session or alternatively by bank transfers made prior to the session. This will be reviewed every August.
COMMITMENT TO SESSIONS AND CANCELLATION

Notice of at least one week is required if you are unable to attend a session or have planned a break.

If you need to reschedule a session in short notice, I have limited flexibility but would try to offer you an alternative time. This is not guaranteed and would need to be taken within 2 working days prior to, or 2 working days following the original session. The full session fee will be charged for sessions cancelled or unattended outside of these terms.
CONTACT

Contact between sessions should be only for the purpose of arranging/rearranging appointments. If you do need to contact me outside our sessions, I can be reached during office hours on 07963 517 330 or by email: info@seanodwyer.co.uk. If I am unable to take your call, please leave a message. Emails, text and telephone messages will be responded to within office hours. I do not communicate with clients on social media or whatsapp. Please contact me only by telephone, email or text message.
CONFIDENTIALITY
Information clients share with me remains private and confidential except in the following circumstances:

If a your safety or safety of others is at risk

I regularly discuss my work with my clinical supervisor. This is standard practice and helps me to work as well as I can with you. Names or any other identifying information is never shared, and my supervisor is bound by the same code of ethics that I am

In the event that I am unable to practice due to accident or sudden incapacity a qualified colleague will contact you and enquire about your wish for further therapy or supervision

If I am compelled by a court of law to disclose notes/information

Non-payment of fees resulting in legal action
I abide by the European Association of Transactional Analysis code of ethics and the UK Council for Psychotherapy Ethical Principles and Code of Professional Conduct
Should any of these points be unclear or cause concern, please raise them in our session.
HOLIDAY/LEAVE
I will inform you of any intended leave well in advance. Please do the same. My dates are listed below and are usually finalised at the beginning of each year so please use this page as a reference. If there are any changes made to these dates I will inform you at the earliest notice.

GDPR – PRIVACY NOTICE
In line with European General Data Protection Regulations, and as a professional registered with the Information Commissioner’s Office, I am obliged to share the following information regarding how your data is protected:
What information do I hold?
As a psychotherapist in private practice I collect the following information:
• Client name
• Address
• Date of birth
• Phone number
• Email address
• GP details
• Records of attendance and payment
What I do with this information.
The information you give when we contract to work together is transferred from paper to my computer where it is encrypted and stored securely.
Email addresses and telephone numbers of current clients are stored securely on my telephone which is backed up onto my computer. Both of these devices are password protected. Only information which would be needed to contact you, for example about appointments, is stored in this way. My computer is not used by anybody else. Your information is deleted after our work has come to an end.
Names and telephone numbers of current clients are shared with my professional executor in line with good practice in the profession. This person would contact you in the event of my death or being unable to work and unable to contact you myself. These details are kept in an encrypted cloud based, password protected account and are updated regularly and deleted when necessary.
I will use your telephone number, email or address to contact you regarding appointments or to reply to you if you make contact with me. I will never share your data with a third party unless obliged to do so by a court of law. In this unlikely event I would inform you first.
